Street address The street address of the entity that will sign the trademark license agreement. Please include Street, Number, Postal code, City, Country.

Web page A general or a palm oil-specific corporate web address.E.g.: 'www.mycompany.com', 'www.mycompany.com/palmoil.html'

Contact name Name and title of a person who has knowledge about the trademark license agreement. E.g.: 'John Doe, Mgr Sustainable Resourcing'

E-mail address Valid e-mail address of the contact person from the previous question.E.g.: 'john.doe@mycompany.com'. The RSPO will use this e-mail address during and after the approval procedure to provide relevant information on sustainable palm oil.

Certificate link Valid online web address of a pdf document with the relevant RSPO supply chain certificate, or a web page with links to several of such pdf documents. E.g.: 'www.mycompany.com/palmoil/certificates/scc24324.pdf', 'www.mycompany.com/palmoil/certificates'. Retailers not required to be supply chain-certified: please write 'Retail: not applicable'.
